PROOF OF IDENTITY, DATE OF BIRTH AND US DURATION OF STAY/STATUS
You must provide at least one of the documents, as listed below, to satisfy proof of full name, date of birth and US duration of stay/status.
Non-US citizens must have at least six (6) months duration of stay in the US reflected on your INS issued document (i.e., Passport, Visa, etc) or possess an INS Notice of Action letter reflecting application for extension of duration of stay and/or visa status is being processed.
- Foreign Passport w/visa and I-94 reflecting duration of stay
- Foreign Passport w/visa, I-94 and I-20
- Foreign Passport w/visa, I-94 and I-66
- Foreign Passport w/visa, I-94, US State Dept Letter (reflecting no diplomatic immunity) and an employment verification letter
- Employment Authorization Card
- Permanent Resident Card
- Permanent Resident Alien Card
